Abstract: Cervical cancer is the second leading cause of cancer-related death worldwide and in Ecuador. Its primary cause is the Human Papillomavirus (HPV); the disease is often asymptomatic, and women frequently show low concern regarding routine check-ups. It progresses silently, progressively attacking cervical cells and posing a serious health risk, particularly to sexually active women. Two vaccines are available globally to help prevent HPV, protecting against genotypes 6, 11, 16, and 18. This study identified a significant prevalence of HPV genotype 58 and analyzed its association with cytological lesions and epidemiological factors. The study was conducted across provinces in Ecuador's coastal region, utilizing a total of 299 samples; Guayas province contributed the largest number of samples. Samples were analyzed using the Anyplex II HPV28 kit (by INPI personnel from the Human Papillomavirus Project), and the results were processed using Minitab 17 statistical software. The analysis revealed that 92% of the samples tested positive for HPV, while only 8% tested negative. Among the positive samples, 82% showed the presence of HPV genotype 58. Associations were identified between cytological lesions and epidemiological factors, corroborating findings from existing scientific literature regarding multiparity, sexual promiscuity, and other relevant variables. HPV genotype 58 was analyzed in association with the other genotypes identified in this study; higher rates of co-occurrence were found with genotypes 16 (38.40%), 31 (30.60%), 33 (28.20%), 35 (36.60%), 53 (43.10%), and 70 (48.60%). A high-risk, high-impact association (p < 0.005) was observed between genotype 58 and genotypes 16 and 70. The results indicated that the co-presence of these three genotypes carries a higher prevalence for causing Cervical Intraepithelial Neoplasia grade III (51%), followed by cervical cancer (17.2%).
