| Abstract: | Severe acute respiratory infections (SARI) cause the death of nearly 4 million people annually, making it the sixth leading cause of mortality in all age groups and the leading cause of death in children under 5 years of age. On the other hand, Ecuador's dependence on foreign technology to implement diagnostic tests implies high costs and a long import time. Considering the importance of epidemiological surveillance and the benefits of a rapid and safe diagnosis, the need arises to develop locally a method for the detection of viruses that cause SARI in Ecuador. The objective of this project is to implement a local multiplex diagnosis of respiratory viruses using the RT-qPCR technique for its application in public health. To this end, primers and probes were designed to detect Influenza A/B, RSV and SARS-CoV-2; then, optimization tests were performed in individual and multiplex reactions to finally validate the test with positive samples. The multiplex PCR simultaneously detected the 3 targets in a single reaction tube and the results with positive samples coincided 100% with the expected diagnosis, demonstrating that it is feasible to use it as a diagnostic method. The local development of the multiplex offers better access to a diagnosis for respiratory infections, in addition to increasing research and improving technological capacity in order to obtain sanitary sovereignty in Ecuador. |
| Description: | Las infecciones respiratorias agudas graves (IRAG) ocasionan anualmente la muerte de cerca de 4 millones de personas, esto la ubica como la sexta causa principal de mortalidad en todo rango de edad y es la primera causa de muerte en niños menores de 5 años. Por otra parte, la dependencia de Ecuador por tecnología extranjera para implementar pruebas diagnósticas implica elevados costos y un demorado tiempo de importación. Teniendo en cuenta la importancia de la vigilancia epidemiológica y los beneficios de un diagnóstico rápido y seguro, surge la necesidad de desarrollar localmente un método para detección de virus que causan IRAG en el Ecuador. El objetivo de este proyecto es implementar un diagnóstico multiplex local de virus respiratorios mediante la técnica de RT-qPCR para su aplicación en salud pública. Con ese fin, se diseñó iniciadores y sondas para detectar Influenza A/B, RSV y SARS-CoV2; luego, se realizaron pruebas de optimización en reacciones individuales y en multiplex para finalmente validar la prueba con muestras positivas. En la PCR multiplex se detectó simultáneamente los 3 objetivos en un solo tubo de reacción y los resultados con las muestras positivas coincidieron al 100% con el diagnóstico esperado, demostrando que es factible emplearla como método diagnóstico. El desarrollo local de la multiplex ofrece un mejor acceso hacia un diagnóstico para infecciones respiratorias, además permite aumentar la investigación y mejorar la capacidad tecnológica con el fin de obtener soberanía sanitaria en el Ecuador |
